Sun Prairie. PAL NEEDED TO START THE CASE. 11 year old girl that’s full of personality and sparkle, and even though she can’t express it with words, it will only take a few minutes for you to fall in love with her chatty blinks and adorable smile. It’s been four years since her accident, in which she sustained a severe traumatic brain injury, and for the first time we’re looking for home nursing to help with her care. We’re looking for specific care during the hours of 6-10 am, to help with her morning trach cares, respiratory routine, and to get her ready for the day while we also get ready for work and get her younger brother ready for school. However, we’re also interested in other hours as well, possibly overnights, starting at 10 or 11 pm, or even going to school with her. As we embark on this journey, we want to be really flexible in finding the best possible care and support for her. Connection with her will be the second most important thing, second only to her safety. A good nurse for her will be energetic, loving, comfortable with caring for tracheostomies, and creative. She uses a wheelchair to get around, and is fully dependent on us for all of her cares and mobility. She loves Taylor Swift, her dog and kitty and brother, facials and anything spa related, and being around people she loves. Thanks so much for your time and interest in caring for our beautiful girl!

Oshkosh.  IMMEDIATE NEED! Night nursing needed for an adult non-vent male. Client is a nonverbal wheelchair bound pleasant/happy man. He has a seizure disorder, G-tube, J-tube who requires tube feedings, suctioning, nebs and CPT vest. He has a mechanical ceiling lift in his room. Shifts available are 7p-7a on Sunday, Wednesday and Thursday one week then Wednesday, Thursday, Friday and Saturday the opposite week. This is a well-organized case with an excellent and fun team of nurses.

She requires trach care and uses a trilogy vent, so you must be vent certified and have an updated CPR(BLS). She has G/J tubes for feedings and meds. She attends the UW Adaptive Fitness program weekly during school sessions. Nurse assists with home therapy program during the day and provides all personal cares. Uses electrical Hoyer for transfers. Colby. Family in search of a PAL to develop, submit, and manage a Forward Health PA and skilled private duty nurses to care for a medically complex child in his home school environment while his parents work. The hours will primarily be weekdays, though it may develop into needs for evening/night and/or weekend hours in the future. Eli, 11 y/o boy with a complex seizure disorder resulting in intractable epilepsy (related to his condition of Lennox Gastaut Syndrome). He is on a ketogenic diet and requires rescue seizure medications intermittently. He has central apnea and intermittent oxygen dependence; with risk for exacerbation when seizure medication increased or rescue medications given. In addition, he has impaired airway clearance, moderate asthma, and chronic bronchitis, which has led to episodes of respiratory failure that can come on very quickly, especially with the onset of illness. He requires skilled nursing assessments to determine the need for escalated respiratory support including chest physiotherapy, cough assist, suction nebulizer treatments, medications and titrating oxygen in the home. In addition, to recognize when he may be in need of treatment changes, medical attention or hospitalization. Eli also has a complex feeding routine due to the ketogenic diet, intermittent electrolyte imbalances, and dysphagia which requires close assessment for safe feeding. Eli is a very sweet boy with developmental delays and wheelchair dependence: who loves to participate in school and fun activities when well. Goal of the family is for Eli to be able to be cared for at home as much as possible and remain out of the hospital.

Janesville. A 31 yr old man who has Duchenne MD. Completely wheelchair bound on the vent with a trach and G-tube. No cognitive delays. Very personable. He has his own apartment that is attached to his parent’s house. He has a Hoyer lift. I need a nurse who will help me get him to bed at night. He uses a cough assist and suction device and may need to use these machines during the night. He is able to advocate for himself and let you know his needs. He may need some minor repositioning during the night.  Needs: one night shift for every Saturday night 10:30 pm to 8:30am.  Hours may be able to fluctuate.  Great working environment. Only dependable need to apply. The hours would need to be billed through KPI Agency and I have contact information for them and can help you get set up with that agency if needed. The rate of pay is currently $45 per hour.
