Tony is fun loving and a sweetheart. He is ambulatory yet ventilator dependent. This is a no lift case. G-tube fed, 37 yr old, cognitively around 3rd-5th grade. Tony takes pride in being a helpful and kind person.
Per Tony’s nurse: “Awesome case, no lifting-can walk, talk, and even dance!! Lovely client, he likes to interact with nurses. The PAL and Mom are extremely helpful with Forward Health and Sandata paperwork/computer skills. Mom is especially helpful for schedule changes and nurses having time off for vacation. Nurses all work well together and I have enjoyed working here. The case PAL is a wonderful resource, and paperwork is always promptly updated to state. Oconomowoc. IMMEDIATE NEED. Vent certified RN/LPN needed for home health care, for a 43-year-old male with cerebral palsy. Skilled in G/J tube, tracheostomy cares, Mediport cares, CPT/nebulizer tx. Experience required. Please call for pre-interview. Hours available: weekends, 12hr shifts and PRN for fill-in, overnight shifts also available. Serious inquiries only please. New Berlin. 7-year-old girl. SMA type 1, treated. Presents like strong type 2. Very smart and active. BIPAP dependent night/nap/illness, no trach. G/J tube. Cough assist, percussion vest, suction. Generally stable case. Lifting required. Uses power wheelchair and has accessible van. Attends full day of school and appointments with her nurses. Must be pet friendly. We are an established case with a great PAL. Looking for evening and every other weekend coverage. No overnights required.
