Tony is fun loving and a sweetheart. He is ambulatory yet ventilator dependent. This is a no lift case. G-tube fed, 37 yr old, cognitively around 3rd-5th grade. Tony takes pride in being a helpful and kind person.

The PAL and Mom are extremely helpful with Forward Health and Sandata paperwork/computer skills. Mom is especially helpful for schedule changes and nurses having time off for vacation. Nurses all work well together and I have enjoyed working here. The case PAL is a wonderful resource, and paperwork is always promptly updated to state.
